However, recovery of consciousness from vegetative state is not always associated with substantial changes in global metabolism. This led us to hypothesize that some vegetative patients are unconscious not just because of a global loss of neuronal function, but rather due to an altered activity in some critical brain regions and to the abolished functional connections between them. We could show that the most dysfunctional brain regions in vegetative patients are bilateral frontal and parieto-temporal associative cortices. Yet, despite the metabolic impairment, external stimulation still induces a significant neuronal activation in vegetative patients as shown by both auditory and noxious stimuli. However, this activation is limited to primary cortices and dissociated from higher-order associative cortices, thought to be necessary for conscious perception. Finally, we could show that vegetative patients have impaired functional connections between distant cortical areas and between the thalami and the cortex and, more importantly, that recovery of consciousness is paralleled by a restoration of this cortico-thalamo-cortical interaction.

Detecting residual cognitive function in persistent vegetative state.

Despite converging agreement about the definition of persistent vegetative state, recent reports have raised concerns about the accuracy of diagnosis in some patients, and the extent to which, in a selection of cases, residual cognitive functions may remain undetected. Objective assessment of residual cognitive function can be extremely difficult as motor responses may be minimal, inconsistent,...

Minimally Conscious State: Evolution of Concept, Diagnosis and Treatment.

The study of consciousness disorders is a scientific challenge, and clinical differentiation among the various sorts of alterations in consciousness is difficult. Persistent vegetative state was defined in 1972, but years later cases appeared in which diagnosed patients showed signs of cognitive activity, and therefore could not be considered vegetative. Minimally conscious state was defined in...

Cerebral response to patient's own name in the vegetative and minimally conscious states.

BACKGROUND A challenge in the management of severely brain-damaged patients with altered states of consciousness is the differential diagnosis between the vegetative state (VS) and the minimally conscious state (MCS), especially for the gray zone separating these clinical entities. OBJECTIVE To evaluate the differences in brain activation in response to presentation of the patient's own name ...

Functional MRI in disorders of consciousness: advantages and limitations.

PURPOSE OF REVIEW We discuss recent developments in the use of neuroimaging and, in particular, functional MRI, in the assessment of patients diagnosed as vegetative state or minimally conscious state. RECENT FINDINGS In the last year, there has been a substantial increase in the number of research studies published which have used state-of-the-art neuroimaging methods to assess residual cogn...

Brain function in brain death, coma, vegetative state, minimally conscious state and locked-in syndrome

We review the nosological criteria and functional neuroanatomical basis for brain death, coma, vegetative state, minimally conscious state and the locked-in state. Functional neuroimaging is providing new insights into cerebral activity in patients with severe brain damage. Measurements of cerebral metabolism and brain activations in response to sensory stimuli using positron emission tomograph...
